These forms are required for each player:
Parent Pledge (Kids Zone)
Commits to supporting positive sideline behavior.
Concussion Information Sheet
Acknowledges concussion risks and removal protocols. Required by California law.
Sudden Cardiac Arrest Information Sheet
Acknowledges SCA risks and warning signs. Required by California law.

Sign the forms and upload through the digital form submission portal.

Reimbursement Requests
Volunteers can request reimbursement for approved expenses using the reimbursement form.
